Working in partnership with each patient's primary care provider, we help address both new and ongoing concerns while ensuring care is coordinated across the healthcare team.

We collaborate closely with patients, families, caregivers and referring providers to manage medications, navigate complex health challenges and develop personalized strategies to support healthy aging. Through ongoing communication, follow-up and coordinated care with the primary care team, we help patients maintain their health, function and independence as their needs evolve.


Our Services

Our geriatric medicine team provides specialized services that work together with a patient's primary care provider and other specialists to address the complex health concerns that can accompany aging. Through coordinated, comprehensive care, we help older adults navigate changes in physical health, memory, mobility, safety and overall well-being while supporting their health, function and independence.

  • Comprehensive Geriatric Assessment and Co-Management Services available at multiple University Hospitals locations throughout Northeast Ohio.
  • Medical House Calls Program for homebound older adults living in private residences, independent living communities or assisted living facilities.
  • Falls Prevention Clinics to help identify risk factors and reduce the risk of falls.
  • Inpatient Geriatric Consultation Services for hospitalized older adults with complex health needs.
  • Post-Acute and Skilled Nursing Facility Care for patients recovering from illness, injury or hospitalization.

Care Across Multiple Settings

As health needs change over time, our team provides care in the setting that best meets each patient's needs. Depending on the situation, care may be provided in an outpatient clinic, at home, in the hospital or in a skilled nursing facility.
