- Scheduling Appointments
To help provide timely service to our patients, we prefer to schedule all visits. If you are late for your appointment, we will try to work you back into the schedule, or perhaps offer an appointment to you with another provider. Out of courtesy to our other patients, we may be forced to ask you to reschedule your appointment.
Occasionally, despite our best efforts, we may run late. We will keep you informed so that you have the option of rescheduling your appointment or seeing another provider, if available.
All visits require an appointment except in case of an emergency. In the event of a non urgent walk in we may ask you to return later in the day when the schedule allows or fit you in to the existing schedule if possible.
- Forms & Policies
Please allow 3-5 business days for medical forms (for school, sports or camp) to be completed. Forms must be dropped off at the front desk during normal business hours or faxed anytime. Your portion of the form should be filled out completely, including your child’s name and date of birth. If the form requires a doctor’s signature, the child must have had a physical within the last year or it cannot be signed. Please remember to schedule your child’s physical yearly.
Forms for School, Sports or Camp

- After Hours/Emergency Calls
The UH Rainbow Call Center provides after hours services.
If an emergency should arise, please call 911. Emergencies are considered to be those conditions that are life-threatening: loss of consciousness, severe bleeding, seizure, etc. In the case of minor emergencies (lacerations, cuts, mild fractures, sprains), please call our office first for instructions.
- Pre-natal Visits/New Babies
Our prenatal visits are by appointment and cost free. Our new baby visits are also by appointment. All of our providers are accepting new patients.
- Sick Visits
We make every effort to schedule appointments for ill children on the day that you call us. Please call our office ahead of time so that we can minimize the time that you will spend waiting. If you have a medical emergency during office hours, we will do our best to see your child immediately or refer you to the appropriate facility.
If you would like us to see more than one child, please schedule both of them when you make your appointment. Without an appointment, we may not be able to see a sick sibling right away, and you may be asked to wait while the provider sees a scheduled patient who may be waiting.
- Well Child Visits/Sports Physical
To keep your children healthy, it is important to bring them in for regular checkups. We recommend that you call our office 2 weeks in advance so you may choose the date that best fits your needs.
UH MyChart allows us to e-prescribe any prescriptions at your appointment within seconds. This will allow us to keep better track of all medications you are currently or have been taking, even if prescribed by another office. For prescription refills please try to give as much advance notice as possible; feel free to leave a message for the nurse, we ask that for most refills your child's physical is up-to-date.
Medications that are more closely regulated by the FDA have different requirements for prescribers. We are required by law to see your child regularly to monitor medication use and usefulness.
