Schedule a Pre-Travel Health Consultation
You can receive your pre-travel medical consultation either by visiting one of our locations or doing so virtually, from the comfort of your own home.
Simply fill out the form below to schedule a pre-travel consult. An administrator from the travel medicine program will contact you to discuss your visit options (in person or virtual) and schedule your appointment time.
Your pre-travel virtual medical consultation will include the following services:
- Itinerary review
- Immunizations
- Travel examinations and consultation
- Review of current medications
- Educational information for destination-specific medical needs
- High-risk medical planning, if necessary
If you schedule a virtual appointment, a follow-up appointment for vaccines (some of which may be covered by your insurance provider) may be required. Payment for the consultation and any non-covered vaccines is expected via credit card at the time the appointment is made.
Please note: You must be physically located in Ohio for a virtual consult.
