UH Meds PGY1 Community-Based Pharmacy residents support patients in achieving their health goals through comprehensive medication management across the continuum of care. Residents work within primary care practices alongside interdisciplinary healthcare teams to develop individualized care plans, optimize medication therapy, and improve access to medications. They also play an integral role in transitions of care through dispensing, discharge counseling, Meds-to-Beds services, and specialized experiences across the University Hospitals health system.
Administrative support is available from UH Meds Patient Support Advocates (PSAs) for insurance verification, prior authorizations, and financial assistance programs, allowing residents to focus on direct patient care and clinical decision-making. UH Meds ambulatory and retail pharmacy services continue to grow and innovate, supporting patients throughout their journey from the inpatient to outpatient setting.
Residents in this program serve on the front lines of patient care, practicing at the highest level of their training and licensure while making a meaningful impact on patient outcomes.
