Premium Care Made Affordable
Patients pay an annual fee to be enrolled in the UH Select concierge primary care program. Office visits may require a copay at the time of service, and our office will bill your insurance for services not covered by the annual membership fee. We accept all types of insurance, including commercial insurance, Medicare and HMO plans.

- Consistent, ongoing preventive care to maintain and improve your overall health, including an annual comprehensive exam and routine blood tests.

A registered dietitian will evaluate your current dietary habits, food selections and supplement use and provide you with feedback and recommendations to help optimize your health and meet your personal nutrition goals.

Through our partnership with T3 Performance, a premier athlete training provider, members may receive a comprehensive fitness evaluation to assess areas such as strength, endurance, flexibility, speed and more. The certified athletic trainers at T3 will work with you to identify ways to help you reach your fitness goals.
