Women’s Cardiovascular Health Group Medical Visits

A new way to help reduce your cardiovascular risk

Our Lifestyle Medicine Group Medical Visits for Women's Cardiovascular Health provide a supportive, empowering space to learn how simple, everyday habits can play a meaningful role in reducing the risk of heart disease – the leading cause of death in women in the US. Led by a Lifestyle Medicine physician or advanced practice provider and health coach, these sessions are grounded in care focused on the American Heart Association’s Life’s Essential 8 including nutrition, physical activity, sleep, stress management, and other daily habits that influence cardiovascular health. You’ll connect with other women who share similar goals while gaining tools and guidance you can use in daily life. These visits are designed to complement – not replace – your cardiologist, medications, or current treatment plan, helping you strengthen your heart health alongside the care you already receive.

Program Details

4 Shared Group Medical Visits with Cohort Participants

  • Led by an Advance Practice Provider (APP) and certified Health and Well-Being Coach
  • Up to 10 patients per group to support and learn with

5 Group Health and Well-Being Coaching Sessions

  • Led by a certified Health and Well-Being Coach
  • Support for implementing behavior change between group visits

Group visits are billed through insurance. Copays, co-insurance, and deductibles may apply. There is no additional fee associated for group health coaching when a part of the group medical visit program.
