Group Medical Visits
A new way to support your health after pregnancy
Congratulations on your new baby! If you had gestational diabetes during pregnancy – or if you live with Type 1 or Type 2 diabetes – the weeks after delivery are an important time to focus on your health, too. Many women with gestational diabetes return to normal blood sugar after giving birth, but about half develop Type 2 diabetes later in life. Early action can help prevent it. This group will address how food, movement and sleep affect your energy, mood and healing, and teach you how to manage your overall well-being and self-care.

Program Details
4 Shared Group Medical Visits with Cohort Participants (Virtual)
- Led by an Advanced Practice Provider (APP) and certified Health and Well-Being Coach
- Up to 10 patients per group to support and learn with
2 Group Health and Well-Being Coaching Sessions (Virtual)
- Led by a certified Health and Well-Being Coach
- Support for implementing behavior change between group visits

- Session 1: Nutrition for breastfeeding and nourishing meals for blood sugar balance
- Session 2: Quick, realistic exercise ideas for busy new moms
- Session 3: Stress management and sleep recovery
- Session 4: Emotional well-being and self-care

Group visits are billed through insurance. Copays, co-insurance, and deductibles may apply. There is no additional fee associated for group health coaching when a part of the group medical visit program.
