Lifestyle Medicine Group Medical Visits for Cardiovascular Health
A new way to help reduce your cardiovascular risk
Our Lifestyle Medicine Group Medical Visits for Cardiovascular Health offer a supportive, empowering space to learn how simple, everyday habits play a meaningful role in reducing the risk of heart disease—the leading cause of death for men and women in the US. Led by a Lifestyle Medicine physician or advanced practice provider and health coach, these sessions are grounded in care focused on the American Heart Association’s Life's Essential 8 including nutrition, physical activity, sleep, stress management, and other daily habits that influence cardiovascular health. You’ll connect with participants who share similar goals while gaining tools and guidance you can use in daily life. These visits are designed to complement—not replace—your cardiologist, medications, or current treatment plan, helping you strengthen your heart health alongside the care you already receive.
To register, please ask your doctor to place a referral. Group medical visits are billed to insurance. For additional information, call UH Connor Whole Health at 216-285-4054.
