Group Medical Visits
A new way to support your health after breast cancer
Breast cancer survivorship is about more than finishing treatment – it’s about healing, rebuilding and reducing future risk. Our Group Medical Visits for Breast Cancer Survivors provide a supportive, empowering space to learn how lifestyle medicine can play a meaningful role in recovery and long-term health. Led by a physician and health coach, these visits are grounded in evidence-based care focusing on nutrition, physical activity, sleep, stress management and other daily habits that influence cancer outcomes and overall well-being. Connecting with other survivors while learning practical, sustainable strategies helps participants feel informed, supported and active in their ongoing health journey.
To register, please ask your doctor to place a referral. Group medical visits are billed to insurance. For additional information, call UH Connor Whole Health at 216-285-4070.
Program Details
4 Shared Group Medical Visits with Cohort Participants
- Led by Physician and certified Health and Well-Being Coach
- Learn and give/receive support with up to 10 patients per group
5 Group Health and Well-Being Coaching Sessions (Virtual)
- Led by a certified Health and Well-Being Coach
- Support implementing behavior change between group visits

Group visits are billed through insurance. Copays, co-insurance, and deductibles may apply. There is no additional fee associated for group health coaching when a part of the group medical visit program.
