Transition from pediatric to adult care for patients with bleeding disorders can be a daunting process for parents and young adults. Planning and Preparation

Transition of care is a process that occurs over time. Planning and preparation for this transition involves providing parents and adolescents with the knowledge and skills needed to achieve successful transition.

At UH Rainbow Babies & Children’s, we believe that transition planning is essential for success. We begin the transition planning process early during the school-aged years to allow for complete preparation as patients transition to adult care.


Transition to Adult Care Providers

Many topics are covered when preparing adolescents to transition to adult care providers, including:

  • Education about individual diseases, treatments, complications and preventative health.
  • Information about insurance coverage and options after age 18.
  • Assistance with obtaining a primary care provider.
  • Continuing education.

Lifespan Transition Support

Lifespan transition support is for our adult patients over the age of 22. We assist patients with information and referrals that can help promote healthy aging strategies for optimal physical, psychosocial and emotional health. This includes resources on:

  • Family planning
  • Prevention
  • Finances
  • Social connections
  • Physical and mental well-being
