Clinical Pharmacy Program

The University Hospitals UH Meds Clinical Pharmacy Program supports patients with complex disease states and medications. Our ambulatory care pharmacists work closely with you and your healthcare team to manage your long term health conditions, helping to adjust your medications and make sure your treatment is working well.

Our pharmacists are an extension to the care you receive at your UH provider's office.

What can your clinical pharmacist do?

  • Collaborate with your UH provider on medications needed for diabetes, cholesterol, heart failure, high blood pressure, weight loss, asthma, COPD, kidney disease and more.
  • Help you understand your medications and answer questions.
  • Discuss financial options and assist in reducing barriers with medication affordability.

How it works:

  • Your healthcare provider refers you into our clinical pharmacy program.
  • You’ll receive a call from a support team member in the days after your provider refers you into our program to arrange an appointment. Appointments can happen via telephone, virtually or in person – you can choose the option that best fits your schedule.
  • After your initial appointment, your clinical pharmacist will determine the frequency of follow-up appointments needed.
  • All information is shared electronically between your primary care provider and the clinical pharmacist through UH MyChart, allowing for ongoing collaboration regarding your healthcare.

“I am very grateful for the help I received from the UH Pharmacy team! My son needed treatment for which the insurance company denied him for two and a half months. I did not know where to find help. After sharing my frustrating situation with my UH PCP, he immediately put me in contact with the clinical pharmacist who supports his UH practice. I quickly received a call from the clinical pharmacist to discuss the problems I was facing and to see how he could help. The clinical pharmacist then worked his “magic” by contacting who he needed to, provided the insurance with proper information and by the next week my son was approved. I am thankful that UH offers clinical pharmacy support free of charge to their patients.”

“At my recent visit with my UH PCP I became tearful when she reviewed my medications to refill as I have to switch insurance and found out that I will not be able to afford this particular medication. She told me about the clinical pharmacy program offered by UH and asked if she can put in a referral, free of charge, to work with a clinical pharmacist. I immediately agreed and my experience working with a clinical pharmacist was AMAZING! Not only did he assist with getting my medication at a much lower and affordable cost, he took the time to review my medication list and was able to convert some others, in consultation with my PCP, to more affordable options. The clinical pharmacist was extremely patient working with me to understand my healthcare needs. He is a wealth of knowledge and I could tell he truly wanted to help people.”
