Clinical Pharmacy Program
The University Hospitals UH Meds Clinical Pharmacy Program supports patients with complex disease states and medications. Our ambulatory care pharmacists work closely with you and your healthcare team to manage your long term health conditions, helping to adjust your medications and make sure your treatment is working well.
Our pharmacists are an extension to the care you receive at your UH provider's office.
What can your clinical pharmacist do?
- Collaborate with your UH provider on medications needed for diabetes, cholesterol, heart failure, high blood pressure, weight loss, asthma, COPD, kidney disease and more.
- Help you understand your medications and answer questions.
- Discuss financial options and assist in reducing barriers with medication affordability.
How it works:
- Your healthcare provider refers you into our clinical pharmacy program.
- You’ll receive a call from a support team member in the days after your provider refers you into our program to arrange an appointment. Appointments can happen via telephone, virtually or in person – you can choose the option that best fits your schedule.
- After your initial appointment, your clinical pharmacist will determine the frequency of follow-up appointments needed.
- All information is shared electronically between your primary care provider and the clinical pharmacist through UH MyChart, allowing for ongoing collaboration regarding your healthcare.

I was nervous because in the past my insurance wouldn’t cover this medication, but my PCP told me I would be working with the clinical pharmacy program to help with cost, adherence and track my progress. They were able to get my medication approved in less than a week. My clinical pharmacist was very helpful with teaching me how to take my shots and how to manage any side effects. He helped me feel less nervous about potential side effects and explained to me how the medication works. This also helps me stay on track and motivated since I am checking in with someone regularly. Our Story
Created in 2019, UH Meds, LLC offers a range of pharmacy services designed to support a patient’s medication needs in the outpatient setting. These services include the management and delivery of retail medications via UH Retail Pharmacy; the supply of complex specialty medications through UH Meds Specialty Pharmacy; the administration of infusion medications at our state-of-the-art UH Meds Outpatient Specialty Care Infusion Center and the home delivery and management of infusion medications through UH Meds Home Care Pharmacy. By extending the array of pharmacy offerings in the community, UH Meds helps our patients streamline their medical care as we carry out our mission of exceptional service, superior outcomes and highest values.
