Employer Authorization Form for Occupational Health Services

In order to treat your employees at a UH Occupational Health Services Clinic, we must receive a completed and signed Employer Authorization Form from any patient visiting us for treatment of a first-time injury or illness. We cannot treat patients without the form.

iSYSTOC User Agreement

Before accessing our Occupational Health client portal, iSYSTOC, you should sign, scan and return the user agreement to OCCHealth@UHhospitals.org. Once we receive, you will receive an email with your username, temporary password and other instructions for accessing iSYSTOC.
