Additional Required Activities
- Greater than 90% of goals and objectives achieved (ACHR is conferred when the resident has achieved the objective at least twice or achieved once with at least two satisfactory improvements noted)
- The remaining 10% must include at least one satisfactory improvement
- Satisfactory completion of all assigned rotations
- Completion of a research project
- Completion of a manuscript for publication
- Formal presentation and/or poster at an appropriate conference
- Four formal internal presentations (grand rounds, faculty teaching, etc)
- Four journal club presentations
- Prepare or revise at least one drug class review, monograph, and guideline
- Completion of one medication use evaluation
- Completion of one performance improvement project
- Facilitate at least one medication safety committee meeting
- Facilitate at least one formulary/policy committee meeting
- Completion of PGY2 required topics
- Attendance at two required conferences (e.g. ASHP Midyear Clinical Meeting and identified opportunity if available)
- Completion of a mentorship program
- Completion of ASHP Medication Safety Certificate
