PGY2 Pediatric Pharmacy Residency at UH Rainbow Babies & Children's
Graduation Requirements
In order to obtain a certificate from the program, the following must be completed by the end of your residency year:
Required Sign-Offs (General)
- PGYl Completion Certificate uploaded to PharmAcademic
- Licensed pharmacist in the state of Ohio within required timeframe
- Complete Pediatric Advanced Life Support (PALS) Training
- Greater than 80°/o of goals and objectives achieved for residency (ACHR)
- 0% Needs Improvement at the time of graduation
- 100% Rl Patient Care ACHR
- Precept and evaluate at least one pharmacy student on APPE rotation or PGYl resident (R4.2)
- Completed PGY2 Pediatric Appendix
- Upload signed PGY2 residency certification to PharmAcademic

Staffing - Longitudinal Deliverables
- Completion of at least 12 weekends staffed
- Completion of staffing checklist (Rl.1.3)
- Pharmacy newsletter with clinical article (R4.l.2)
Research Longitudinal - MUE Deliverables
- Complete medication use evaluation (MUE)
- Present a poster at a national meeting (R2.2.6)
- Present MUE locally at appropriate hospital meeting (R2.2.4)
Research longitudinal - Major Research Project Deliverables
- Research project completed
- Project proposal submission (R2.l.4)
- Research project protocol defense presentation (R2.2.2)
- Research project presented at a national meeting (R2.2.6)
- Research project manuscript final draft due 6/1 (R2.2.S)
Didactic Requirements - longitudinal Deliverables
- Complete 1 new, pediatric drug monograph OR update 2 adult drug monographs with pediatric information and present at MST (R2.1.1)
- Complete and present a journal club, journal scan, formal case presentation, informal and interactive case report, seminar, preceptor development, and grand rounds presentation (R4.l.l, R4.l.2, R4.1.3, R4.l.4, R4.l.6)
Manager On call - Longitudinal Deliverables
- Complete on-call log with reflections (R3.l.l, R3.l.2, R3.2.2, R3.2.3)
Evidence Based Practice and Quality Improvement - Longitudinal Deliverables
- Complete the EBP Scholar program and obtain the EBP certificate
- Utilizing GRADE tables, develop recommendations for development of a clinical practice guideline (R2.l.l)
- Collaborate with the EBP group to develop a grade table for a PICO question (R2.2.3)
- Present findings from EBP at a Clinical Pharmacy Specialist meeting (R4.1.4)
Medication Safety - Deliverables
- Medication safety newsletter (R4.1.S)
Neonatal Intensive Care Unit (NICU) - Deliverables
- Pharmacist newsletter-style written education with assessment questions (R4.1.5)
Pediatric Intensive Care Unit (PICU) Deliverables
- Educational activity designed for PICU medical or house staff (R4.1.3)
