Collaborative Care at University Hospitals Behavioral Health Institute

Through this program, behavioral health clinicians from the UH Behavioral Health Institute work in our primary care offices to diagnose and treat behavioral health issues onsite whenever possible. As part of APC, each patient is assigned a behavioral health manager, who works directly with a psychiatrist to help manage their care. This collaboration is especially beneficial for patients who may have to wait weeks or months to see a psychiatrist.

We’re expanding our integrated, collaborative care model to primary care practices throughout the UH system.

Mental Health Care Through APC

After you establish care with an APC provider, a mental health professional will be available at the same location to discuss any behavioral health concerns you may have.

Your discussion may include:

  • Your symptoms, mental health history, treatment history and how to optimize your mental health.
  • Brief talk therapy and education to help with your symptoms.
  • Medication management, when needed.

Your behavioral health manager will communicate with your PCP and other mental health specialists to develop a plan-of-care designed specifically for you. Going forward, this integrated team will continue to work together to manage your mental healthcare needs.

Psychological and behavioral health issues for which APC patients can receive help include:

  • Depression
  • Anxiety
  • Bipolar disorder
  • Addiction and substance abuse
  • Grief counseling
  • Trauma disorders
