Signing Up for the UH Personal Health Record

The UH Personal Health Record (PHR) is a safe and secure way to access your medical records and office visit notes online, communicate with your physician, request appointments and prescription refills, pay your bill and much more. University Hospitals partners with a vendor called FollowMyHealth™ to provide you with access to your medical records.


We’re making it even easier to sign up! Beginning on February 19, all patients 18 and older with an email on file will receive an email invitation to sign up for a PHR.

The invitation will be automatically sent following any encounter you may have with University Hospitals, including inpatient stays, visits to your primary care physician or specialists, and after laboratory or imaging tests/procedures.

You just have to open the email and follow the simple instructions provided and you’ll be on your way to enjoying the benefits of an online personal health record.

Currently, you can continue to sign up online for a UH PHR from your mobile device or computer or at your UH provider’s office.

You can sign up for your UH PHR from the comfort of home anytime using a cell phone, tablet or computer. You must be at least 18 years of age to sign up for your own PHR. To enroll a minor (a patient under the age of 18), please visit the Parent/Guardian access page to learn more.

Sign Up at Your Provider’s Office

You can sign up for a UH PHR at any UH physician office or UH hospital at your next appointment. If you select this method, you will need to visit the UH location in person and bring a photo ID to verify your identity. Provide the front desk staff member with your email address and ask that they send you an invitation.
