Signing Up for the UH Personal Health Record
The UH Personal Health Record (PHR) is a safe and secure way to access your medical records and office visit notes online, communicate with your physician, request appointments and prescription refills, pay your bill and much more. University Hospitals partners with a vendor called FollowMyHealth™ to provide you with access to your medical records.
INVITATION EMAIL SENT AUTOMATICALLY AFTER UH VISIT
We’ve made it even easier to sign up! All patients 18 and older with an email on file will receive an email invitation to sign up for a PHR. Please make sure to provide your current email address at the time of check in.
The invitation will be automatically sent following any encounter you may have with University Hospitals, including inpatient stays, visits to your primary care physician or specialists, and after laboratory or imaging tests/procedures.
You just have to open the email and follow the quick instructions to accept the invitation and you’ll be on your way to enjoying the benefits of an online personal health record.
If you do not have an upcoming appointment, you can sign up online for a UH PHR from your mobile device or computer.
