Going home after a hospital stay is certainly something to look forward to. But the transition from hospital to home can be stressful. Your medical team will discuss your home-going plan with you and your family every day up to discharge to help ensure a smooth process.

Discharge plans vary from individual to individual. Please alert your healthcare team to any concerns you have about going home as early as possible. For instance, if you may need transportation, let us know as soon as you can. Most discharges occur by noon.

University Hospitals Cleveland Medical Center’s conveniently located Bolwell Pharmacy is available to fill your discharge prescriptions.

Satisfaction Surveys

Patient satisfaction surveys help us better understand our patients’ needs and improve our service to all patients. A survey may be mailed to your home once you have been discharged, and we would very much appreciate you taking a few minutes to complete it.

Patient and family education is a shared responsibility between patients, families and caregivers. Your healthcare providers can address topics such as healthy lifestyles, risk-reducing behaviors, developmental needs, activities of daily living and preventive self-care. Please talk with your nurse about any patient education or discharge planning information you think you may need.
