UH Future Nurse Academy Program Application
Instructions:
- Complete all sections of the below online application. Please note, you cannot save your work and return later. This form needs to be filled out in one sitting, but it should take less than 10 minutes.
- Obtain parent/caretaker permission; they should be filling this form out with you.
- Email ALL requested documentation to UHFutureNurseAcademy@UHhospitals.org:
- Confidential Reference/Recommendation
- Student COVID-19 Vaccination Card
- W-9 Form
- Request Confidential “Reference Assessment/Form” (not including parents). We must receive a complete recommendation form for your application by April 21, 2023.

Applicants will be notified of the Selection Committee’s decision. If selected, you and your parents/legal guardians will need to attend mandatory Parent/Student Orientation and UH Future Nurse Academy training (dates TBD).
APPLICATION DUE DATE: April 21, 2023, by 11:59 p.m.
We will inform applicants of their acceptance/non-acceptance status by May 6, 2023.
Confidential References
Your application requires that we receive One Confidential Recommendation, one character reference from someone who knows you well (inside or outside of school) or from a teacher. This individual should be able to highlight reasons why you would make a good healthcare professional one day. All recommendations should come directly from the person who is providing your recommendation. Ideally, the responses will highlight the following:
- For how long and in what capacity they have known you, the applicant
- An evaluation of your leadership potential, including strengths and/or weaknesses
- Characteristics that distinguish you from other candidates
NOTE: References completed by family members will NOT be accepted. All references should come directly from the person providing your recommendation. Any recommendations sent from the student or from family members will not be accepted.
How to submit your reference letter
Please ask your reference to send their recommendation letter to UHFutureNurseAcademy@UHhospitals.org with your name in the subject line on or before April 21, 2023.

"Hello Mr./Ms./Mrs. _______,
I am applying to the UH Future Nurse Academy, and I would be honored and appreciative if you'd complete a recommendation for my application. I'm asking you because I really enjoyed and learned a lot in your class, and I think you have an excellent sense of me as a student and person.
According to the program application, recommenders need to complete an online assessment that highlights the following:
- For how long and in what capacity you have known me, the applicant
- An evaluation of my leadership potential, including my strengths and/or weaknesses.
- Characteristics that distinguish me from other candidates.
The recommendation letter must be turned in no later than April 21, 2023, by 11:59 p.m. to the following email: UHFutureNurseAcademy@UHhospitals.org. I appreciate your consideration!"
University Hospitals support the recommendations put in place by the CDC and the science of using vaccinations. As future healthcare providers, we encourage you to take the first steps towards caregiving by aiding us in protecting your fellow UH Future Nurse Academy program participants, the staff members of University Hospitals, and the patients we serve.
All students interested in participating in the UH Future Nurse Academy must be fully vaccinated, which requires two doses of an approved vaccine (e.g., Pfizer, Moderna, Novavax, or Johnson and Johnson). Please email an image of the front and the back of your vaccination card to this email: UHFutureNurseAcademy@UHhospitals.org
- I am fully vaccinated
- I am NOT fully vaccinated, but plan to get vaccinated prior to the program starting
