Applications Forms

To apply for the 2026 UH Future Nurse Academy, begin by filling out the student application. Please note that you will need the following before you begin:

• A pen and paper - you can save your application and return to complete it later, but you will need to remember your return code
• Your parent or guardian's phone number, email and signature
• One letter of reference from someone who knows you well but is NOT a family member
• Copy of your health insurance card
• Copy of your childhood vaccinations. This can be a statement from a physician or a copy of all records.

You can save your application and return to it at any time, however YOU MUST write down the return code to get back to the form you started; without this code, you will need to start all over.
