UH Future Nurse Academy Mentor Program Application

Instructions:

1. Must be 18 years or older to apply.
2. Current NAII/NAIII at UH in good standing.
3. Complete all sections of this application.
4. Send the completed application along with approval to participate from UH manager
5. Applicants will be notified of the Selection Committee’s decision. If selected, you will need to attend mandatory Mentor Training and the Student/Caregiver Orientation Night (dates TBD).

We will inform applicants of their acceptance/non-acceptance status by May 6, 2023.
