Our 50-bed, state-of-the-art acute inpatient rehabilitation hospital is dedicated to the treatment and recovery of individuals who have experienced a variety of conditions including:

UH Rehabilitation Hospital provides acute inpatient medical and functional rehabilitation. We offer patients three hours of therapy per day, five days a week.

Outpatient services for individuals with conditions that do not require inpatient rehabilitation care are available at several convenient community locations.

Multidisciplinary Team Develops Individual Treatment Plans

A physiatrist, a doctor who specializes in physical medicine and rehabilitation, will lead the clinical team in developing an individual treatment plan to meet the needs of each patient and family. Internal medicine physicians are also available to provide medical consultation and management to oversee the medical needs of each individual patient. In addition to physicians, therapists and nurses, there will be a case manager available to assist each patient with length of stay management and discharge planning.

Each patient’s treatment team will be uniquely tailored to meet his or her needs, and may also include a combination of the following specialists:

Helping Patients Smoothly Transition back to Community

UH Rehabilitation Hospital offers a transition to community (TTC) apartment that assists in preparing patients, families and caregivers for a safe community re-entry. Our TTC apartment is a home-like environment inclusive of a fully equipped kitchen, bathroom and features that are normally found within a home.

The goal is to facilitate a well-prepared return-to-community that increases the patient’s safety while reducing anxiety, risk for re-injury and future hospitalizations.
