The UH Requester Portal allows third parties to request access to the medical records, subject to the terms and conditions.

Who Can Use the Portal

This portal is not intended for UH patients. Patients can request their medical records via their UH MyChart account or through a records release form. Applicable third parties may include:

  • Attorneys
  • Government officials
  • Other non-patient requesters or patient proxies

University Hospitals Requester Portal Terms and Conditions of Use

The University Hospitals Requester Portal website ("UH Requester Portal") is provided by University Hospitals Health System, Inc. and its affiliates (collectively, "UH" or "Health Care Providers") to third parties requesting access to the medical records of the patients of UH subject to compliance with the terms and conditions set forth below (the "Agreement"). Please read the following information carefully. Your continued use of the Requester Portal will indicate your agreement to be bound by the terms and conditions of this Agreement. If you do not agree to be bound by this Agreement, promptly exit the Requester Portal.

  1. Requester Portal.
    The Requester Portal is an Internet application that enables you to have web-based access to request information from the medical record of a patient maintained by UH in electronic format.
  2. Access.
    Your Requester Portal account will be accessible by using your email address and utilizing multi-factor authentication to complete the login process. This process must be used prior to every visit to the Requester Portal. Once you have access, you will be asked to submit a Release Authorization Form to UH’s Health Information System (“HIS”) Department. Upon receipt of a properly executed Release Authorization Form, HIS may request additional information before releasing the requested medical record. Upon approval by HIS, a copy of the medical record will be released to the email used to access the Requester Portal. UH reserves the right to request any additional information necessary to verify the validity of the request prior to release. It is your responsibility to prevent further disclosure of the patient’s medical record once it has been released to you. UH is not liable for any damages that result from your failure to maintain the confidentiality of such medical record once so released.
  3. Privacy.
    By accessing or using the Requester Portal you agree to be bound by all the terms and conditions of this Agreement and UH's Privacy Policy incorporated herein by reference to available at this link. It is important to review the Agreement and Privacy Policy periodically as we may modify this Agreement and the Privacy Policy at any time, and you agree that such modifications are effective and binding upon you immediately upon the posting of the modified version.
  4. Information Does Not Constitute Medical Advice.
    You agree that the Requester Portal is a communication service offered only as a convenience tool. UH assumes no responsibility for how you use the information you obtain from the Requester Portal. The information from the patient’s medical record available through UH Requester Portal may or may not constitute your entire medical record, depending on the scope of your request. THEREFORE, YOU AGREE TO ASSUME THE SOLE LIABILITY FOR YOUR ACTIONS USING UH REQUESTER PORTAL AND SHALL HOLD UH HARMLESS AND DEFEND UH AGAINST ALL DAMAGES RELATED TO SUCH USE OR THE SUBSEQUENT USE OR DISCLOSURE OF SUCH INFORMATION AFTER THE DOWNLOAD OR TRANSMISSION OCCURS. FURTHERMORE, YOU AKNOWLEDGE AND AGREE THAT UH SHALL NOT BE LIABLE FOR ANY DAMAGES, PERSONAL INJURY, INCLUDING DEATH, ARISING FROM YOUR USE OR MISUSE OF UH REQUESTER PORTAL OR ANY INFORMATION OR CONTENT THEREIN.
  5. Restrictions on Use and Ownership.
    The medical record provided to you through the Requester Portal is a copy of the requested patient’s medical record. UH owns the underlying medical record itself. The content on UH Requester Portal is provided by UH as a convenience only. The words "University Hospitals Health System, Inc." and its associated corporate logo are the trademarks and service marks of University Hospitals Health Systems, Inc. All other trademarks, service marks, and logos used in UH or UH MyChart, including but not limited to MyChart and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on UH MyChart without the express permission of UH or the third party that may own the trademark, service mark, or logo.
  6. Termination of Access.
    UH may restrict or terminate access to UH Requester Portal at any time and for any reason. Access may be restricted from time to time for planned maintenance of the UH system. If your use of UH Requester Portal violates this Agreement, UH reserves the right to permanently terminate your access to UH Requester Portal.
  7. Payment.
    Following confirmation of the accuracy of the Release Authorization Form, but prior to releasing the requested medical record, you must pay the fee for the requested medical record. The fees are posted on UH’s website located here https://www.uhhospitals.org/patients-and-visitors/medical-records/fee-schedule. Payment may be made through the Requester Portal.
  8. Security.
    Once information enters UH Requester Portal it is protected with industry standard encryption software. No system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using UH Requester Portal, information will be transmitted over a medium that is beyond the control of the UH and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED ACCESS AND/OR DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION, OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE AND AGREE THAT THE UH SYSTEM IS NOT ERROR-FREE.
  9. Third Party Links and Content.
    UH makes no representations whatsoever about any other web site that you may access through UH Requester Portal. When you access a non-UH site, please be advised that it is independent from UH, and that UH has no control over the content on that web site. UH does not endorse or accept any responsibility for the content or use of any other linked site. If you decide to access any third party sites linked to UH Requester Portal, you do so entirely at your own risk. There may also be material in UH Requester Portal that is provided by third parties. UH is not responsible for any such third party material. UH disclaims any responsibility for or liability related to such third-party materials and your use of any such third party materials.
  10. No Warranties.
    UH Requester Portal is provided on an "as is" basis for your convenience. UH makes no representations or warranties of any kind, express or implied, as to the operation of UH Requester Portal, the content, products and/or services included therein. To the fullest extent permitted by law, UH hereby disclaims all warranties, express, or implied, including, but not limited to, implied warranties of merchantability, fitness for a particular purpose, title, and/or infringement.
  11. Applicable Law.
    Your use of UH Requester Portal is governed by Ohio law. By using UH Requester Portal, you agree that any claim or dispute relating to your use of the same, regardless of where you reside, shall be subject to the exclusive jurisdiction of the local, state, or federal courts located in Cuyahoga County, Ohio.
