Referring Patients for Nutrition Referrals
Registered dietitian nutritionists at the Digestive Health Institute work hand-in-hand with referring physicians and multidisciplinary healthcare teams to deliver coordinated and cost-effective care. Our registered dietitian nutritionists are skilled in providing medical nutrition therapy (MNT). Our registered dietitian nutritionists monitor and evaluate the patient’s progress and provide nutrition education for disease prevention and nutrition counseling for chronic conditions.
To refer a patient for medical nutrition therapy, call 216-844-1499. It is preferred that patients are referred through the ambulatory electronic medical record (aEMR). However if needed, a patient can also be referred by completing and faxing one of the forms below:
