Population Health Management with Epic Healthy Planet

Focusing on the health of patient populations is crucially important to the advancement of medical care and research. It improves the clinical treatment of specific groups by promoting better patient outcomes and lower costs for delivering care. Implementing the Healthy Planet module of the Epic electronic health record (EHR) at UH will be a critical step toward our mission to prevent illness, optimize health and improve the quality of life for our patients and our community as a whole, while increasing the efficiency of our providers. Epic’s Healthy Planet module features integrated tools to help caregivers monitor a patient’s care over time. This enables them to take action to maintain a patient’s health or improve chronic conditions – no matter where they receive care. Healthy Planet dashboards show which patients need follow-up care and demonstrate key quality initiatives. Providers can more readily order follow-up for groups of patients all at once, rather than one at patient a time. Predictive analytics provide a risk score for patients that not only identifies their percentage of risk, but also provides details about what specific factors attribute to that score. Caregivers are then able to enroll patients into interventions that will address their rising risk scores.
