What to know before entering PT/OT Wheelchair evaluation referrals:

  • Insurance will only cover power wheelchairs or scooters for use inside the home.
  • The patient must have a mobility limitation that significantly impairs their ability to perform their ADLs within the home.
  • The patient must have a long-term need for the chair.
  • The patient must have a diagnosis that impairs body functions which limit mobility (neurological, musculoskeletal, cardiopulmonary, etc.).
  • For most Medicare insurance products, the patient must have a face-to-face visit with the physician. The visit note must detail the patient’s impaired mobility, impaired ADL, and be the primary reason for the visit.
  • The referring physician must sign the therapist’s evaluation once completed and provide documentation to the Durable Medical Equipment supplier.
  • Schedule Me Now will not display appointment availability for seven days due review of insurances, coordination of vendor.

What to know before entering Physical Therapy-aquatic referrals:

  • Only one referral is necessary to enter in the AEMR. Provider does not need to enter an additional Physical Therapy general referral, as therapist will assess patient on land. Evaluating therapist will determine amount of aquatics vs. land therapy on case-by-case basis, but pool therapy will be included in intervention per provider referral.
  • Contraindications:  Incontinence of bowel/bladder, open sores, drainage, bandaging or illness. Consideration of patient history such as high blood pressure, seizures, blood clots. Therapy will follow up with ordering physician as needed.
