Our kidney care navigator services are available for patients with advanced chronic kidney disease. The navigator provides expert patient support and can assist by:

  • Communicating with patients to determine needs and concerns
  • Helping patients to manage and attend appointments and complete various assignments
  • Troubleshooting possible challenges or barriers to care and help to find solutions

In addition, the kidney care navigator can provide patients with education and guidance on:

  • Chronic kidney disease/end-stage renal disease
  • Hemodialysis evaluation/placement
  • Anemia management
  • Dialysis initiation
  • Kidney transplant evaluation
