Birth Certificates & Social Security Numbers

You must record your baby’s name with the Cleveland Bureau of Vital Statistics. To help you do this, your nurse will give you a Hospital Birth Certificate Worksheet form to fill out and return before discharge. A fee is involved; call the Bureau at 216-664-2315 for details.

By checking the appropriate box on the Birth Certificate Worksheet, you may also obtain a Social Security number for your baby. You will receive the card and number in about four months. There is no fee involved in obtaining a Social Security number. Do not offer to pay any company or person to obtain a social security number; this is illegal.
