Providing Acute Care for the Elderly (ACE)

University Hospitals Center for Lifelong Health offers Acute Care for the Elderly (ACE), an innovative concept that was developed at University Hospitals Cleveland Medical Center in 1994. This intervention program offers multidisciplinary geriatric care using a patient-first approach to fully assess patients as they prepare to return home after a hospital stay.

Patient-Centered Care

At the Center for Lifelong Health, we take a patient-centered approach to interventional care, offering services such as:

  • Interdisciplinary team rounds
  • Plans for the patient’s return home
  • Prescribed environment for patient preferences

Multifaceted Team Approach

The ACE intervention team at UH includes:

  • Dieticians
  • Floor nurses
  • Geriatricians (physician specialized in caring for older adults)
  • Gerontologic clinical nurse specialists
  • Pharmacists
  • Rehabilitation therapists
  • Social workers
