Patient Navigation Help Individuals Manage the Complexities of Sickle Cell Disease

The Adult Sickle Cell Disease Clinic at University Hospitals Seidman Cancer Center offers the benefits of a multidisciplinary clinic coordinated by a patient navigator. The patient navigator helps people with sickle cell disease manage this chronic condition.


Patient Navigator Coordinates Care

Our highly effective patient navigator helps patients navigate emotional, physical, and practical, day-to-day barriers to care.

The navigator:

  • Supports and educates patients
  • Assists patients in scheduling appointments
  • Arranges transportation if needed
  • Provides reminders and follow-up calls for patients

The patient navigator can help patients get to their appointments, ease their stress around healthcare, and improve the quality of patients’ lives.


Multidisciplinary Clinic

Our Multidisciplinary Adult Sickle Cell Disease Clinic allows patients to meet with several different specialists and sub-specialists on the same day. In addition to seeing the sickle cell disease experts, patients will be able to meet with specialists in medicine and supportive care.

Transition Clinic for Pediatric Patients

The adult sickle cell team holds four transition clinics throughout the year, together with experts in pediatric sickle cell disease from University Hospitals Rainbow Babies & Children’s Hospital. This clinic helps ease the transition of adolescent patients from pediatric sickle cell management to the Adult Sickle Cell Disease Clinic.
