A History of Patient Success at Home at University Hospitals Parma Medical Center

Since 1984, UH Parma Medical Center home health care has helped thousands of patients maintain independence in their homes. Our professional staff serves as a link between patient and physician, working with individuals to reach their optimal level of health and independence by delivering care and needed services, including:

  • Nursing: Services include medical and post-surgical care, infusion nursing and wound or care of stoma sites with an ileostomy or urostomy.
  • Physical therapy: Including evaluation and treatment, services emphasize activities such as bed mobility, transfers, walking, stair climbing and assessment of durable medical equipment such as wheelchairs, home modification for safety and exercise instruction.
  • Occupational therapy: Medical evaluation and treatment focuses on improving arm/hand function, cognitive/perceptual abilities, developmental conditions and/or daily living skills.
  • Speech/language therapy: Provided for patients with deficits in speech, language, hearing, reading, writing, memory and thinking skills, as well as swallowing problems.
  • Home health aides: All aides are state certified to assist with personal care services such as daily grooming or monitoring conditions.
  • Social worker: Provides assistance with community and financial resources.

Nursing services are available seven days a week, including weekends and holidays. Evening visits can be made as necessary.

Eligibility for Home Health Care Services

To receive home health care, patients must:

  • Be under the care of a physician, with written approval for home care
  • Be homebound with a medical condition that prevents leaving the residence on a regular basis
  • Participate in the treatment plan and cooperate with health care personnel

A Fully Credentialed Program

UH Parma Medical Center home health care is Medicare certified and accredited by The Joint Commission, as well as licensed by the Ohio Department of Health. In addition to Medicare and Medicaid, we accept private insurance and private payment for eligible services.
