At UH Elyria Medical Center, our home care services can also help patients learn to live with their illness or disability. Our goal is to help individuals regain their independence by becoming self-sufficient and maintaining optimum health.
We offer a wide array of home care services, including:
UH Elyria Medical Center Home Care, along with Valued Relationships, Inc. (VRI), assists patients to easily monitor their health and vital signs with a home telemonitoring unit called HealthPal. The HealthPal device is used as part of a comprehensive disease management program, which monitors the patient’s vital signs and symptoms, then sends the information to the home care nurse provider. This early warning system enables the nurse to resolve small problems before they escalate, leading to fewer emergency room visits and hospitalizations.
