Center for Comprehensive Rehabilitation (CCR)

The Center for Comprehensive Rehabilitation (CCR) at University Hospitals Lake Continuing Care Center is an acute rehab facility that helps patients regain independence after a serious illness or injury. A doctor may recommend acute rehab in the CCR before a patient returns home. A CCR stay is a separate stay covered by Medicare and most insurance companies. Our goal is the patient’s safe return to independent living.

The 12-bed inpatient rehabilitation unit treats patients age 18 and over who require intensive rehabilitation following a serious illness, injury or surgery, such as:

  • Amputation
  • Bilateral joint replacement (Patients must have other ongoing medical issues that require continued hospitalization and close physician supervision)
  • Brain injury
  • Complex orthopedic surgeries
  • Hip fracture
  • Multiple traumas
  • Neurological conditions
  • Spinal cord injury
  • Stroke

Why Choose CCR?

In contrast to a skilled nursing facility (SNF), CCR provides a more intensive rehabilitation program that provides at least three hours of therapy a day, five days a week. CCR has 24/7 nursing care, and your care team will monitor your progress frequently to keep you on track with your rehabilitation goals. The American Heart Association and the American Stroke Association both strongly recommend that stroke survivors receive acute inpatient rehab treatment at a facility like CCR versus a SNF level of care to achieve the best outcomes.

Before admission, patients will be evaluated to determine their ability to participate and benefit from the intensive rehab program.


Private Patient Rooms

All patient rooms are private with large windows, private baths with roll-in showers and plenty of space for family and visitors. Physical and occupational therapy services are located within the center and feature a spacious gym with access to an outdoor patio. There is also a fully-equipped kitchen and laundry space where patients lean the skills necessary to return home. Patients and family members can also relax and spend time together in a lounge that features a large screen TV, computer, refreshment area and access to an outdoor patio.


Your Team

An individualized treatment plan will be created for the patient by our team of physicians, rehab nurses, physical, occupational and speech therapists, and social works or case managers. Each day, patients will work with a variety of therapists to regain function and skills, from grooming and bathing, to getting in and out of chairs, to improving cognitive skills. Patients typically stay in the CCR about two weeks, depending on their needs, potential and goals. The team will meet twice a week to monitor the patient’s progress. We also recognize the important role family plays in the patient’s recovery. We involve family members throughout the rehabilitation process and train them to meet any special needs at home.
