Signing Up for the UH Personal Health Record

The UH Personal Health Record (PHR) is a safe and secure way to access your medical records and office visit notes online, communicate with your physician, request appointments and prescription refills, pay your bill and much more.

University Hospitals partners with a vendor called FollowMyHealth to provide you with access to your medical records.

Email Invitation to Sign Up After UH Visit

All patients 18 and older with an email on file will receive an email invitation to sign up for a PHR. Please make sure to provide your current email address at the time of check in.

The invitation will be automatically sent following any encounter you may have with University Hospitals, including inpatient stays, visits to your primary care physician or specialists, and after laboratory or imaging tests/procedures.

You just have to open the email and click on the link provided to accept the invitation and you’ll be on your way to enjoying the benefits of an online personal health record.

If you do not have an upcoming appointment, you can sign up online for a UH PHR from your mobile device or computer.

You can sign up for your UH PHR from the comfort of home anytime using your cell phone, tablet or computer. You must be at least 18 years of age to sign up for your own PHR. To enroll a minor (a patient under the age of 18), please visit the Parent/Guardian access page to learn more.

You can also sign up by downloading the FollowMyHealth or UH Now mobile app on your iPhone or Android device.

If you are not able to successfully sign up online and you do not have any upcoming appointments, please visit your provider’s office, show a picture ID, and ask to fill out the PHR Paper Enrollment Form.
