How to Refer Patients
The Vascular Center at University Hospitals Harrington Heart & Vascular Institute in Cleveland, Ohio, has streamlined different points of entry for patients in order to facilitate expedient and efficient care.
This system is designed with the referring physician in mind as we prioritize physician requests during processing. Our referral schedulers are happy to accept calls and answer any queries that referring physicians may have.
To refer a patient, please have the following patient information available:
- Patient’s full name
- Date of birth
- Social security number
- Current address
- Current phone number(s)
- Insurance information
- Presenting symptoms
- Medical history
If using an outpatient referral form, please ensure that it contains the required information above.
The call center at the Vascular Center assigns the time and date of a patient’s appointment when the physician calls to make a referral. Similarly, our referral management assistants provide the physician or the physician’s office with this information when our division office is contacted.
Online requests are directed to our office, and we will contact the referring physician with appointment information at the earliest opportunity.
