Coming in October - UH Personal Health Record (PHR) to replace UH Samaritan Medical Center Patient Portal
Welcome to the UH Samaritan Medical Center Patient Portal, the online tool that gives you the flexibility to access your health information and other resources on your time, from any device with internet access, and between visits to University Hospitals Samaritan Medical Center. You can also use the UH Samaritan PHR to access information for family members and individuals for whom you provide care if given permission.
Starting on October 5, the UH Samaritan Medical Center Patient Portal will officially transition to MyUHCare PHR, the University Hospitals system-wide personal health record platform. This will enable current users of the existing UH Samaritan Patient Portal to:
- See medical results from additional UH facilities
- Use only one login
- Enjoy a better, more streamlined experience
What do I need to do if I am a UH Samaritan Patient Portal User?
The current UH Samaritan Patient Portal will no longer be available after November 15.
If you have a UH Samaritan Patient Portal account today, you will need to take the following two actions:
- Obtain paper copies from Medical Records (419-207-2552) or print/save your records from the current UH Samaritan Patient Portal.
- Register for MyUHCare PHR.

Visit www.uhhospitals.org/HowToSignUp to sign up for MyUHCare, the University Hospitals system-wide personal health record.
MyUHCare PHR Enrollment Fairs
Would you like some in-person assistance? UH Team Members will hold enrollment fairs from September through November to answer questions and assist with registration. Visit the MyUHCare PHR table on the following dates:
UH Samaritan Main Lobby, 8:30 a.m. – 3:30 p.m.
- September 5
- October 4, 14, 29
- November 13
- Ashland County Fair, 10 a.m. to 5 p.m., UH Samaritan Medical Center First Aid Trailer
- September 20
