In order to obtain a certificate from the program, the following must be completed by the end of your residency year:

  1. Licensed pharmacist in the state of Ohio within the required timeframe
  2. Greater than 80% of goals and objectives achieved for residency
    • Zero needs improvement at the time of graduation
    • All of R1 Patient Care goals achieved for residency
  3. Research project completed  and presented at a national meeting
  4. Precept and evaluate at least one pharmacy student on APPE rotation or PGY1 resident during one month rotation
  5. Present at least four presentations
  6. Present a poster presentation at a national meeting
  7. Complete Pediatric Advanced Life Support training (PALS)
  8. Complete medication use evaluation (MUE) and present locally at appropriate hospital meeting
  9. Complete Topic Appendix required by ASHP
