Requirements

In order to obtain a certificate from the program, the following must be completed by the end of your residency year:

1. Licensed pharmacist in the state of Ohio within required timeframe
2. Greater than 80% of goals and objectives achieved for residency (ACHR)
   • 0% “Needs Improvement” at time of graduation
   • All R1 Patient Care ACHR status
3. Research project completed
   • Must be presented at a national meeting
   • Manuscript due 6/1
4. Precept and evaluate at least one APPE student/PGY1 resident
5. Present at least four presentations
6. Present a poster at a national meeting
7. Complete medication use evaluation and present locally at an appropriate meeting
8. Complete Pediatric Advanced Life Support (PALS) Training
9. Complete ASHP topic appendix
