Colleen Zelonis, CNM, MSN, has been a practicing certified nurse midwife since 2006, preceded by more than 25 years’ experience as a labor and delivery nurse. She received a master’s degree in nursing from Case Western Reserve University in 2001 and midwifery certification from the Frontier School of Nurse Midwifery and Family Nursing the same year. Known for spending extra time with patients, Colleen’s primary goal is providing safe, competent and compassionate care. She provides care to healthy women during pregnancy, labor and delivery, and postpartum, with available options that include holistic and waterbirths. Colleen also performs Pap tests, breast exams, contraception and family planning options, and treatment of bladder and vaginal infections. She has privileges at UH St. John Medical Center and Fairview Hospital.

Colleen and her husband, Gary, reside in Avon Lake where they have raised their blended family of seven children.

I am a new patient with Colleen Zelonis and she was phenomenal. She was kind, she listened to me and my concerns and she was personable. I will be recommending her to any woman I known in need of a women's health care provider. I am so happy that I chose her as I was in need of a new provider myself.

Colleen Zelonis is great. She delivered my first two boys and I wanted her to deliver my final third boy. She was very helpful and had a lot of information during the pregnancy and she was able to deliver my third boy with no issues.

Colleen is the first midwife I have ever gone to. She is amazing, understanding and willing to get to the answers quickly and efficiently. UH hospitals in general house the best medical providers I have ever seen. I would absolutely suggest everyone I know to be seen by UH medical professionals. They are excellent.

I travel 30-40 min to see Colleen Zelonis, she is wonderful and worth the drive! I have seen Colleen twice for my pregnancy so far and am glad she's my midwife. I felt comfortable with her from day one. You can tell she truly cares about you as a person as well as your health. I don't feel like my visits are rushed and like I'm another patient to get marked off her list, the way I did with another provider. Colleen personally texted me my lab results the next day for my glucose test. When I had issues getting my voicemails returned she personally saw to it that the office manager called me to get my appointments set up (apparently letting people work from home during Covid messed up the lines of communication). She is one of the many reasons why I love being with University Hospitals! I'm excited she does annual female exams and not just pregnancy visits. Hopefully she'll be with UH a long time, I don't want to have to find another provider for my female care.

Industry Relationships

University Hospitals is committed to transparency in our interactions with industry partners, such as pharmaceutical, biotech, or medical device companies. At UH, we disclose practitioner and their family members’ ownership and intellectual property rights that are or in the process of being commercialized. In addition, we disclose payments to employed practitioners of $5,000 or more from companies with which the practitioners interact as part of their professional activities. These practitioner-industry relationships assist in developing new drugs, devices and therapies and in providing medical education aimed at improving quality of care and enhancing clinical outcomes. At the same time, UH understands that these relationships may create a conflict of interest. In providing this information, UH desires to assist patients in talking with their practitioners about industry relationships and how those relationships may impact their medical care.

UH practitioners seek advance approval for certain new industry relationships. In addition, practitioners report their industry relationships and activities, as well as those of their immediate family members, to the UH Office of Outside Interests annually. We review these reports and implement management plans, as appropriate, to address conflicts of interest that may arise in connection with medical research, clinical care and purchasing decisions.
