University Hospitals Accountable Care Organization Improves the Health and Wellness of Employees and Dependents

University Hospitals’ employee accountable care organization, UH Accountable Care Organization (UHACO), provides the infrastructure and experience that is the foundation for UH’s accountable care models. UHACO began by seeking to improve the health and wellness of employees and dependents that are enrolled in UH employee medical plan. In 2010, we established the UH employee accountable care organization for about 25,000 employees and dependents to focus on:

  • Increasing the role of the primary care provider (PCP)
  • Expanding care coordination and disease management processes
  • Promoting wellness and prevention
  • Providing resources for patients to manage their own health
  • Implementing clinical pathways and programs to improve population health

Through the UHACO team of registered nurses, clinical dieticians, and outreach coordinators, UHACO helps our members achieve the highest quality of care throughout this network by providing:

  • Personalized member outreach to address gaps in care
  • High-risk case management services
  • Value-based insurance design analysis and programs
  • Clinical disease management services
  • Care coordination services

We offer a wide-range of wellness programs to help our employees develop and maintaining a healthy lifestyle, including UH OptiWeight™, a weight management program developed and led by UHACO board-certified clinical dietitians.

Employee ACO and the UHACO Network

Quality patient care starts with the most important health care relationship — a primary care provider, or PCP. UHACO members are incentivized to select a PCP for all members covered under a UH medical plan. Our UHACO PCPs partner with members and the UHACO to:

  • Increase preventive care
  • Address routine health concerns in the appropriate care setting
  • Educate patients on health and wellness opportunities
  • Facilitate appropriate disease management
  • Increase adherence to evidence-based guidelines for chronic conditions
  • Coordinate care for members across the continuum of care

UHACO is working with the centers of excellence within University Hospitals Health System to develop tailored evidence-based guidelines for its members’ care through the UHACO Network.
