Cost & Enrollment

Members of UH Select pay an annual membership fee to have access to our concierge primary care services in northeast Ohio.

Membership is offered on an annual basis, but you can cancel the program at any time, although the annual fee will not be refunded. The membership fee is $2,400 per person, per physician. This fee is out-of-pocket and is not covered by insurance.

For services not covered by the annual fee, our office will bill your insurance provider directly. A copay may be required for some office visits. Members can use FSA and HSA funds to cover copays and any other out-of-pocket expenses.

Enrolling in UH Select

UH Select is open to all persons 3 years old and up. To join UH Select, simply call 216-844-4YOU (216-844-4968) and we will help setup a meet & greet with one of our providers.
