Senior Adult Assessment Program
If you're concerned about frequent falls, memory lapses, depression, or difficulty managing home life-either for yourself or someone you love----the Senior Adult Assessment Program at UH Extended Care Campus can help.
The Senior Adult Assessment Program provides a one-day, comprehensive assessment for the older adult. Our goal is to assist older persons, their families and primary care physicians to identify special health care needs and provide optimal, individualized solutions.
In a warm and caring environment our highly skilled team, including a physician, psychologist, physical therapist, nutritionist and social worker, will meet with your loved one. An occupational therapist visits the home to assess the level of safety there. Within two weeks, our team meets with you and your family member. You are presented with written recommendations which will give you the knowledge, tools, and resources to plan a safe future.

Older adults, their family members, primary care physicians, and caregivers.
