Referrals to UH Harrington Heart & Vascular Institute's Anticoagulation Monitoring Service

To enroll in our clinic, patients need a referral and the active participation from their physician. The physician should provide:

• A completed referral form (PDF)
• A recent office note and/or discharge summary

Anticoagulation clinic services are typically covered by insurance plans and Medicare. Patients should check with their insurance company prior to their appointment to ensure coverage for our outpatient hospital services.

To schedule a patient appointment, a referring physician’s office should call 216-844-3800 and then fax a completed referral form (PDF) to 216-844-1780.
