Interdisciplinary Team Approach to Care

The Center for Lifelong Health uses an interdisciplinary team approach to care. A physician, nurse and social worker evaluate seniors for health care recommendations. Patients, family members and caregivers are then scheduled for a summary conference with the team to review the findings, learn the recommendations and ask questions regarding the diagnosis, treatment options and lifelong care. Our compassionate team coordinates plans for follow-up and also provides assistance with referrals for specialists, legal issues and financial matters.
