Parking Permits – Ohio Bureau of Motor Vehicles
To obtain a handicap parking placard or license plate, you must submit an application and a prescription from a physician or chiropractor. The prescription must include your name, the date, the doctor’s signature, and the duration of the disability (can be permanent).
