Applications for the 2009-2010 academic year will be accepted beginning Summer of 2009.
The application deadline will be October 1, 2009. Notification for interviews will occur shortly after October 1.
Required for Fellowship
- Board Eligible or Current Certificate in Pediatrics, Internal Medicine, or Family Practice.
- Applicants must be a U.S. citizen, Permanent Resident, have work authorization, or J1 visa status. The hospital does not support H1 status visas.
- The program will accept the universal application available on the AMMSM website.

Please include the following with your application:
- 3 letters of recommendation mailed directly by the author to the address below. One letter should be from your Residency Program Director.
- A complete list of all Sports Medicine related rotations completed.
- Medical school diploma
- Residency diploma / Certificate of residency completion (if available)
- Copies of state medical licenses
- USMLE or COMLEX scores
- ECFMG certificate (if applicable)
- Medical school dean's letter
- Medical school transcript
