What is a Patient Resume?
The Patient Resume is a document you can fill out to help you tell your child’s story and organize their medical information. Just as a job resume shows all your work-related skills and experience, the “Patient Resume” summarizes your important knowledge about caring for your child. The various pages cover important information including your child’s medical diagnosis, emergency information, medications, and other topics specific to your child’s needs.
How do I use the Patient Resume?
The Patient Resume is designed to help manage whatever information is important to you in the care of your child.
The following steps will get you started:
1. Gather as much of your child’s information as you can.
This may include:
- physical examination records
- recent doctor’s visit notes
- psychological testing
- surgical reports
- insurance cards.
- Contact information for doctors, therapists, teachers and other care providers
2. Find the section where this information belongs and write it in the appropriate place.
(You may not need all sections.)
3. Keep a copy with you to share with each person involved with your child’s care.
4. Update the Patient Resume as you child’s information changes.
Currently, the Patient Resume can be printed out to be used as a handwritten document. A more interactive version that you can fill out and save electronically will be available online soon.
