The Geriatric Assessment Center Evaluates Older Adults for Physical and Mental Function
At University Hospitals Parma Medical Center, we understand that caring for older adults is often a delicate and complex balance of diagnoses and treatment. If your loved one is having difficulty managing the activities of daily living and has experienced a deterioration of physical or mental capabilities, a geriatric assessment could be a valuable next step.
The team of geriatric specialists UH Parma Medical Center can evaluate older adults to establish a comprehensive baseline of daily functionality.
Spouses, adult children and physicians may be prompted to seek an in depth evaluation for an older adult based on the following:
- Caregiver stress and concerns
- Concerns about driving safety
- Decline in physical abilities
- Depression or anxiety
- Driving concerns
- Falls and trouble with balance
- Impaired vision and hearing
- Medication concerns
- Signs of memory loss
What to Expect During a Geriatric Assessment
The patient and/or caregiver will be asked to complete a detailed questionnaire of the patient’s current health status, covering their physical, mental and social wellness. During this thorough assessment, blood work and imaging tests may be ordered. Appointments take an average of two hours.
During the Follow-up Meeting
The physician, nurse and social worker meet with the patient and their caregivers to develop an appropriate treatment plan. This information also is shared with the patient’s primary care physician to ensure continuity of care.
The care team will provide support and education to the patient and their family, including referrals to community and hospital resources as appropriate. Referral resources can include UH Parma Medical Center's The Club for those with mild memory loss and Beyond Driving with Dignity, a driver safety evaluation.
