Most cases of heart failure require day-to-day management by the patient, under the direction of our nurse clinicians in the Advanced Heart Failure & Transplant Center. This caring medical team provides you with nutritional guidelines, exercise instruction and education about your condition and medications. We help you set goals and adjust your lifestyle in small steps that make it easy for you to manage your heart failure.
