How to Share your Health Information
How do I share information in my UH Personal Health Record with hospitals or other providers outside of the UH health system?
To share health information from your UH Personal Health Record (PHR):
- Login to your UH Personal Health Record.
- Open the “My Health” tab to view a summary of your health information.
- Click on the “Export” button.
- Select “Transmit” from the “Export Type” dropdown menu to send all or a portion of your UH Personal Health Record to a hospital outside of the UH health system. You will need to know the hospital’s direct email address. This is different from the hospital’s standard email address.
How do I print, fax or email information from my UH Personal Health Record?
Navigate to the My Health tab. You will see buttons on the page to print, fax or email. Click on the button you need.
Self-Assigned Patient Access
How can I share my health information with my spouse, other family members or caregiver?
Adults can grant access to the information contained in their personal health record to other people such as spouses, caregivers or other family members. This allows the person granted access to perform simple tasks like requesting appointments and refilling prescriptions. Adult users of the personal health record can assign access – either full or view-only – to another person by clicking the My Account drop-down menu and selecting My Connections. Select the arrow next to With Family Members, Dependents, etc. Then click the Add Authorized Individual button and follow the instructions to fill out and submit the form.
