UH Parma Personal Health Record

Coming in October

UH PARMA MEDICAL CENTER HEALTH INFORMATION IS AVAILABLE ON MYUHCARE PHR

We are excited to announce that you will now be able to view your records for UH Parma Medical Center in your MyUHCare PHR. Visit information and results recorded on or after October 6 will be available.

This will enable PHR users to:

  • See medical results from additional UH facilities
  • Use only one login
  • Enjoy a better, more streamlined experience

What You Need to Do if You are a MyUHCare Parma User

The current MyUHCare Parma portal will no longer be available after November 15. After November 15th, records may be obtained by contacting Medical Records at 440-743-4242.

If you have a MyUHCare Parma account today, you will need to take the following 2 actions:

  1. Print/Save your records. Click here for instruction on how to print your personal health record.
    Note: If you would like import your saved document into MyUHCare, click here for instruction.
  2. Register for MyUHCare.

How Do I Sign Up?

Visit www.uhhospitals.org/HowToSignUp to sign up for MyUHCare, the University Hospitals system-wide personal health record.

MyUHCare PHR Enrollment Fairs

UH Team Members will hold enrollment fairs in October and November to answer questions and assist with registration. Visit the MyUHCare PHR table in the lobby from 8:30 a.m. to 3:30 p.m. on the following dates:

UH Parma Medical Center – 7007 Powers Blvd, Parma

  • October 3, 8, 9, 10, 11, 12, 19, 25
  • November 1, 9, 15

UH Wellpointe Health Center – 303 E Royalton Rd, Broadview Hts

  • October 16, 31
  • November 12
