Accessing Your Medical Records
HIPAA (the Health Insurance Portability and Accountability Act) gives patients the right to request access to, inspection of, and receive copies of their medical records. There are certain exceptions to this right, including access to psychotherapy notes.
You have the right to request copies of your electronic medical records in an electronic format. For instance, University Hospitals may provide you with a copy of your radiology results on a CD-ROM.
You may direct UH to send your medical information to another individual. To do so, you must complete the UH Authorization for Release of Medical Information form. UH must act upon your request with 30 days, or give you a written notice explaining why extra time is needed to act upon your request. That notice must tell you the date UH will complete the request, in no case exceeding 60 days.
UH may charge you for certain records at a rate allowed by State law.
